FGF21, a member of the Fibroblast Growth Factor (FGF) family, is a hormone primarily produced and secreted by the liver, which plays a pivotal role in the regulation of glucose and lipid metabolism, as well as body weight management. It enhances glucose uptake in muscle and adipose tissue, suppresses hepatic glucose production, promotes fatty acid oxidation, and reduces lipid synthesis in the liver, contributing to weight management.

Elevated levels of FGF21 have been observed in individuals with obesity, type 2 diabetes, and other related metabolic disorders, as well as in those who are overweight. This elevation has led researchers to speculate that FGF21 may be intricately linked to the pathogenesis of these conditions, making it a potential therapeutic target for their treatment.

Various strategies have been identified to stimulate FGF21 secretion:

  1. Diet: A ketogenic diet, characterized by high fat and low carbohydrate intake, has been shown to increase FGF21 levels. This diet promotes an increase in hepatic fatty acid oxidation and a reduction in glucose production, which in turn boosts FGF21 secretion.

  2. Exercise: Regular physical activity has also been found to elevate FGF21 levels, likely through enhanced fatty acid oxidation in both muscle and adipose tissues.

  3. Cold Exposure: Exposing the body to cold can trigger an increase in fatty acid metabolism, which is associated with higher FGF21 production.

  4. Fasting: Fasting stimulates fatty acid oxidation, which can lead to increased FGF21 secretion.

  5. Medications:

    • Metformin: Commonly used to manage type 2 diabetes, metformin has been shown in some studies to raise FGF21 levels, contributing to its beneficial effects on glucose metabolism.
    • GLP-1 Receptor Agonists: Drugs such as exenatide and liraglutide, used for type 2 diabetes treatment, may increase FGF21 levels, aiding in glucose management and weight reduction.
    • SGLT2 Inhibitors: Medications like canagliflozin and dapagliflozin, also for type 2 diabetes, have shown potential to enhance FGF21 levels, supporting their positive impact on glucose control and weight loss.

While preliminary data suggests that increasing FGF21 levels can have beneficial effects on weight loss and metabolic health, comprehensive research is essential to fully understand how FGF21 influences body weight regulation and to evaluate the safety and efficacy of related therapeutic interventions. Furthermore, given FGF21's multifunctional nature, its role in weight management might be dependent on specific physiological contexts, necessitating further studies to clarify its effects.
